JTC Medical History cum Ezlink Example
Dear JTC campers,
Here is an example of a Medical History cum Ezlink card.
We want a scan of the front of your Ezlink card. Exactly the same size as your card. Nothing bigger, nothing smaller.
Behind your card, you will write down your medical history, as shown in the example below.
1. Your Full Name / Squad
2. Next of Kin Name, Contact, Relationship with you
3. Food/Drug Allergy
4. Last Hospitalisation
5. Past Injuies
6. Triggered by?
7. Medication
8. Others (Information regarding your wellbeing & health that we must know)
* You will water proof your card by scotch taping every single part of the card, both sides as well.
All informaiton to be as detailed as possible. If you have Asthma, tell us when is the last time it occured, if it is dormant or it still happens frequently and also medication required. For any health issues, all medication required MUST be clearly stated. This is to facilitate our First Aid Process, should anything unpleasent happens during camp. Thus, we want the most detailed & accurate information.
All cards are to be ready by this Friday. Uncompleted cards/ Cards that are not waterproofed will NOT be accepted. Thank you.
